The Program

Henry Ford Self-health is a comprehensive program to support patients and caregivers in dealing with a chronic condition.

Our certified Self-health Coaches work one on one in private consultation with our patients. Together, they develop a patient-unique algorithm, or, healthy pathway, to identify and achieve their health goals.

We do this through offering ongoing disease education, nutritional guidance, activity programs, support groups, and innovative products.

Our Centers host a quartet of room vignettes that are designed with functional ideas focused on home safety and healthy living. Home renovation examples by local contractors are on display along with many other items that will assist patients and caregivers live their lives to the fullest.

Through our individualized program, we are able to offer our patients what they desire. One more chance, while helping them maintain their dignity and independence.
